ifference between 74A and 74D is the groups.
AD Groups are meant to group users for a specific task or right.
Management Role Groups are meant to group roles and not users.
If you set it all up with AD groups, it will be easier to manage, because after setting up AD groups you don't need the ECP or PS management to add/remove users anymore.
Evenmore, you can use those AD groups for more tasks for those technicians in the offices later, if you wish. So it's a more flexible solution.
The question asks for the "Best" answer. I find using AD groups the best. :)

Just quick, I currently don't have the time to do good research..
74-D because the scopes should contain the servers in the offices, the AD groups contain the technicians and you need the roles to glue those.
61-D because you need hardware loadbalancers (DNS roundrobin does not solve hardware failures) and connections need to go to the Hub servers because they need ntlm authentication (which is not on Edge servers).
50-A you need a CAS in each site, but one CAS will proxy so users only need to connect to one of them (just a quick guess).
30-C this prevents attempts to external on CAHT02, which is not internet-facing.
